Infants
[Ages Two and Younger]
An individual must have at least one developmental delay in any of the following areas:
- Cognitive development
- Physical development
- Communication skills
- Psychosocial maturation
- Self-help skills; or
- A biological or an environmental risk for acquiring developmental delays.
Pre-school
[Ages Three through Five]
An individual must have at least two developmental delays in any of the following areas:
- Cognitive development
- Physical development
- Communication skills
- Psychosocial maturation
- Self-help skills; or
- An established risk, a biological risk or environmental risk for acquiring
developmental delays
Ages Six through Fifteen
An individual must have a diagnosis of a specific developmental disability, which is characterized by the following:
- The disability is attributed to a mental and/or physical impairment
other than a sole diagnosis of mental illness;
- The disability is likely to continue indefinitely;
- The disability results in substantial functional limitations in at
least three of the six areas of life activity on the Children's Ohio
Eligibility Determination Instrument (COEDI)
- Psychosocial maturation
- The disability causes the individual to require a combination and sequence
of special, interdisciplinary or other type of care, treatment,
ofprovision of services for an extended period of time that is
individually planned and coordinated for the individual.
Ages Sixteen and Older
An individual must have a diagnosis of a specific developmental disability, which is characterized by the following:
- The disability occurs before the age of twenty-two
- The disability is attributed to a mental and/or physical impairment other than the sole diagnosis of mental illness
- The disability is likely to continue indefinitely
- The disability results in substantial functional limitations in at least three of the seven areas of life activity on the Ohio Eligibility Determination Instrument (OEDI)
- The disability causes the individual to require a combination and sequence
of special, interdisciplinary or other type of care, treatment,
ofprovision of services for an extended period of time that is
individually planned and coordinated for the individual.
