Service and Support Administration
The Service and Support Administration Department assists individuals and their families to identify needs and obtain the supports necessary to meet those needs based on the available resources. Service Coordinators work closely with the individual, their family, and other staff to develop and implement services that will enable the individual to acquire new skills as they need and prefer. Service Coordinators are responsible for:
  • Needs assessment;
  • Development of individual service plans;
  • Arranging for services;
  • Monitoring the provision of those services;
  • Assisting in the selection of providers;
  • Incorporating the results of quality assurance reviews into the individual service plan
The Service and Support Administration Department is also responsible for:
  • Serving as an information and referral contact to respond to questions and requests from individuals in the community regardless of their eligibility for services;
  • Providing crisis intervention and necessary emergency services to individuals and their families on a 24 hour basis;
  • Review, investigate and report any Major Unusual Incident, which would include alleged abuse, neglect, or other unusual incidents involving individuals with developmental disabilities.

Information and Referral
Wood Lane has a centralized application process for services. Service and Support Administration is responsible for the intake and eligibility of all applicants with the exception of early intervention and school services.

Early intervention eligibility is determined by the Wood County Health Department. School service eligibility is determined by the student’s local school district. Individuals can apply for services by contacting Service and Support Administration.
